Help! Open wound on common pleco - with pictures

Hello,

we have a 8 year old common pleco in a 120 Gallon tank. He always had a grayish spot on his tail since he was a baby, and it got worse. He is not eating, has a sunken belly.
All other fish in the tank are fine (1bristle nose, ~25 tetras, other small fish).

We do 60% water changes every 3 weeks. Eheim Pro3 filter. Tetra water conditioner Aquasafe plus. San Antonio, Texas.

First treatment: 1 dose API Fungus cure, we had to skip second dose because he appeared not fine and none of the fish were eating. Wound didn't change.

Second treatment: 3 doses Seachem Kanaplex, Kanamycin (according to instructions).

Wound looks red at borders, gets larger, there is some white crust on top with some small protruding thin parts. No visible fungus. Red bubble like structures at borders. Please see pictures.

We plan to treat him with API General cure (praziquantel, metronidazole) next because we don't know what it is.

Please advise, pictures attached.
