My oldest angelfish had been very swollen for several months now. I treated her for worms, parasite and I even tried an epsom salt bath but nothing worked. I decided that as long as she was swimming ok and still eating I would leave her be. Anyway, the other day she took a turn for the worse. Gasping terribly, hiding with her nose up, not eating...it wasn't good. So I put her down. She was the first angelfish that I bought several years ago, so it was really sad saying goodbye to her. Also, my platinum angelfish took a liking to her and stayed with her the entire time and protected her. He even stayed with her when she was hiding and gasping.
I ended up doing a quick autopsy on her because I was really worried that she had internal worms or something. When I opened her up I was shocked at how many eggs she had inside of her...lots of eggs. I also found what looked like a hard gray mass of eggs stuck together. I can't be certain that that's what it was, but whatever that mass was it seemed to be preventing her from expelling the eggs.
